Participating Service Provider Links for Locust Valley CSD (NY)
Where no indicator appears, the Service Provider has not yet signed an Information Sharing Agreement (ISA) with OMNI. OMNI is diligently working with all remaining Service Providers to maximize their participation in your plan. In order for a service provider to participate they must enter into an Information Sharing Agreement with OMNI by January 1st, 2009, or once your employer has a written Plan Document, whichever is sooner.
